Please refer to the recent e-mail you should have received with current forms required to volunteer at Shorehaven:

1) Volunteers must have a current volunteer application on file at Shorehaven with a copy of covid card

2) Volunteers must have a background check form on file at Shorehaven

3) Volunteers must have a TB assessment form on file at Shorehaven

4) Volunteers must have a TB test done at Shorehaven at a scheduled time some time this year

5) Volunteers must wear a mask while on Shorehaven property
